The Terms of Service have been written to protect your rights from potential suppressive action by the FDA to deny you access to obtain nutritional therapy.

This web site is meant for informational and educational purposes only. All information contained within this website, is solely to assist participants in taking charge of their own health. This includes all information relating to diagnosis, treatment and health care recommendations.

The information provided is not intended to substitute for the advice, diagnosis, or treatment of your medical provider. One should always consider to consult their physician or other qualified health care provider for medical advice and/or treatment.

This website should not be used as a substitute for the medical advice or services of your healthcare provider(s).

Agreement to proceed with the Anti-Aging Health Questionnaire constitutes a "contract" between ICNR, Inc. and you the participant as identified by your name and your e-mail address hereinafter referred to as MEMBER and that such contract is based on the following understanding and agreement:

1. Parties believe that the ultimate authority on health is God, and that his instructions for health, along with the substances created by HIM to attend health, are expressed to mankind through scripture, and clearly evident in nature as created by God, therefore, that system of health care referred to herein and promoted by PARTIES as "natural Health", is clearly understood to be a ministry of God.
2. That all individuals are God's children, and therefore have a "God-given" right to seek God's ministry for their health concerns, and to care for their bodies and health, and that of their dependents, as they deem appropriate.
3. That Gods children have a right to come together as a congregation of believers (referred to herein as membership), to learn about Gods plan for health, to share advice and education on scriptural health care and natural substances, and to enjoy and express these rights unimpeded.
4. That individuals (PRACTITIONERS) that choose to study, share and offer advice on Gods instructions for health and His natural substances with other believers, have a right to do so, and that individuals (MEMBERS) that choose to seek the advice and advanced knowledge of PRACTITIONERS, have an equal right to do so.

Therefore, PARTIES, by checking the "I agree" statement to this document, hereby confess their belief and support of this God given privilege to enjoy and express these rights and beliefs, and confer their promise to uphold and defend PARTIES rights to these beliefs and actions, through all legal means. MEMBER attest that he/she is acting solely on his/her own behalf, and/or that of a dependent, and does not, and is not seeking membership for any other reason or purpose, or on the authority or behalf of any governmental or other entity.

PARTIES agree that membership as defined herein is voluntary, and that any of the PARTIES may cancel this agreement at any time without cause, and without notice. In the event MEMBER terminates membership due to dissatisfaction with SERVICES or relationship with PARTIES, or for any other reason, it is hereby understood that the exclusive remedy is the dissolution of membership and relationship with the PARTIES, and that no punitive or compensatory reimbursement or award of any kind will be due MEMBER. If MEMBER should seek reimbursement, award, compensation of other form of satisfaction of any kind, such MEMBER will be deemed to be in violation of the terms of this agreement, and hereby acknowledges that such breach entitles the PARTIES from which recourse is sought, to all remedies under the law for MEMBER breach, including full reimbursement for cost to defend this agreement, along with other damages as may be deemed appropriate.

MEMBER attest that he/she is of sound mind, has read, understands, and agrees to the terms herein, and by checking the "I agree" statement below, expresses his/her rights as declared herein, and as provided in the constitution of the United States. MEMBER chooses to join as a MEMBER of ICNR, Inc. to participate in membership activities, and to receive scriptural natural health SERVICES from the Anti-Aging Health Questionnaire created by Sir, Dr. Gerald H. Smith, who is a Knight Hospitaller and registered/certified practitioner of the World Organization of Natural Medicine, a non-denominational Christian membership based division of the Sovereign Medical Order of the Knights Hospitallers.

Notice: MEMBER understands that services provided by ICNR, Inc. and Sir, Dr. Gerald H. Smith are not defined as "medical", therefore PRACTITIONERS provide no diagnosis or treatment of any disease condition.
